CASE MANAGER​

Full Time Position 

Pay Scale:  $13 - $15 an hour

​

Qualifications:  Master’s degree in a human services field from an accredited college or  university in social work or other human services field and nine credit hours in graduate level courses that focus on family and individual function and interaction. One year of documented full time work experience in a treatment setting serving children, including RTC, child placing agencies providing treatment services, psychiatric hospital serving children etc., or one year of documented full time work experience in a residential child care operation, or related field of child and family services. A bachelor’s degree from an accredited college or university with two years of year of documented full time work experience in a residential child care operation, or related field of child and family services. Any educational or professional qualifications to be considered as equivalent, in lieu of the above minimums, require prior approval of the CEO and Licensed Child Care Administrator or designee. Must be 21 years or older.

Duties:

  • Complete an admission assessment or any other evaluation of child for placement;

  • Participate in resident intake and assessment interviews;

  • Developing, reviewing, maintaining and updating of service plans for residents at SD;

  • Maintain confidentiality for all client and program operations;

  • Complete a discharge or transfer summary for a resident;

  • Approve any restrictions that will be imposed on a resident for more than seven days that have not been reviewed and approved by the service planning team, and any monthly re-evaluations of restrictions that continue for more than 30 days;

  • Approve any restrictions to communication and visitation with the child’s family that are imposed on a child, but have not been reviewed and approved by the service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days;

  • Approve any restrictions to a particular room or building for more than 24 hours that are imposed on a child, but have not been reviewed and approved by the service planning team;

  • Establish helping relationships with clients and families/guardians;

  • Provide advocacy as specified by and contract guidelines 

  • Facilitate and participate in individual and group counseling

  • Maintain professional and ethical standards as prescribed by SD and maintain a professional demeanor at all times

  • Comply with SD policy and procedure manuals

  • Comply with all Licensing Standards as well as funding source guidelines

  • Prepare and submit documentation, assessments and reports within required timelines

  • Provide crisis intervention for client and families 

  • Actively participate in training, workshops and in-services provided by SD or outside agencies as requested

  • Serve as a positive role model to resident

Documenting Responsibilities:  Professional Level Service Provider person will be responsible for assessing, facilitating and monitoring all components of assigned caseloads. The professional level service provider will sign, date and store the following documents to indicate review and approval or disapproval:

  • Admissions assessments or any evaluation of a child placement

  • Initial service plans and updates

  • Discharge or transfer summaries;

  • Approve any restrictions that will be imposed on a resident for more than seven days that have not been reviewed and approved by the service planning team, and any monthly re-evaluations of restrictions that continue for more than 30 days;

  • Approve any restrictions to communication and visitation with the child’s family that are imposed on a child, but have not been reviewed and approved by the service planning team, including monthly re-evaluations of restrictions that continue for more than 30 days;

  • Approve any restrictions to a particular room or building for more than 24 hours that are imposed on a child, but have not been reviewed and approved by the service planning team.

Authority:  The Professional Level Service Provider person's authority is limited to initiating and  carrying out the case management and other duties in the Professional Level Service Provider person job description. The Professional Level Service Provider’s caseload will be manageable to ensure that all needs of SD residents are meet.
